Bloom Community Care

Our Process

Bloom Community care is a Registered Home Aged Care Provider for Australian Citizens aged 65 years and over.

The Australian Government provides Home Care Packages to eligible Australians who need extra care services at home. They are designed for those with more complex care needs that go beyond what the Commonwealth Home Support Program (CHSP) can offer and aim to maximise independence and well-being.

Eligibility is determined through Aged Care Assessment Services (ACAS) and is not affected by your income, if you are receiving Centrelink payments or if you already have a carer.

If you are eligible and depending on the level of care needed you may be offered one of the following:

Package level

Level of care needs

Level 1

Basic care needs

Level 2

Low care needs

Level 3

Intermediate care needs

Level 4

High care needs

You have the right to choose a provider who meets your needs best and puts you in charge of the services you will receive.

Once you have been offered a Home Care Package you will be able to choose a provider who meets your needs best and puts you in charge of the services you will receive.  Once you choose your provider the government then pays your provider a subsidy to arrange a package of care services to meet your needs.

Our management and support staff speak various languages such as English, Arabic, Assyrian, Chaldean and Turkish. All staff hold necessary qualifications and screening checks in order to provide safe and informed services. They will work with you on a day-to-day basis and will always be available to listen to any concerns or suggestions or improvements.

 

EXIT PROCEDURES

If you should wish to end service provision before the dates set out in your Service Agreement, we expect that you will give Bloom Community Care at least one month notice in writing.

You will be provided with information and support to access a person of your choice, such as an advocate, to assist you in exiting Bloom Community Care. Prior to exiting, you will be provided guidance and support to:
• Investigate other options or models of support
• Consider re-entry to Bloom Community Care should your needs or circumstances change within 30 days
• Provide any feedback they wish to give

Your privacy rights will be respected and protected at all times in accordance with the Bloom Community Care Privacy and Confidentiality Policy and Procedure.
